The Jordan Valley Mobile Dental Unit will be visiting our school on November 10-12 and November 17-19. This program is available to students who have Medicaid. A Jordan Valley dentist will lead a professional staff that will provide onsite dental services to eligible students at Forsyth School. Students who qualify will be given an initial exam and dental treatment will be determined by the dentist. Details of the exam and care given, will be available for parents to keep for their records. Parents can register online at www.jordanvalley.org/services/mobile-school-based-care/ or paper packets are available for pick up in the MS Office, HS Office, and Nurse's Office. Appointments are limited. Priority is given to students who have the greatest dental need, as well as those who return the completed paperwork promptly.
