VACCINES AND SPORTS PHYSICALS
All 7th grade students are required to have the Tdap and Meningococcal vaccines before entering the 8th grade. All 11th grade students are required to have a 2nd Meningococcal before entering the 12th grade. For your convenience, the Cox CARE Mobile will be on campus Thursday, April 2 to administer vaccines and sports physicals. If you would like your student to participate, registration forms are available in the school nurse's office. Forms must be returned by Tuesday, March 31.
The Jordan Valley Mobile Dental Unit will be visiting our school beginning April 7. This program is available to students who have Medicaid. If you would like your student to participate, registration forms are available in the school nurse's office. Return the completed paperwork promptly.
